PLEASE NOTE – When applying for a phone from the Tom Howat Memorial Hardship Fund this must include a referring agency detail i.e., Charity, CIC, NHS/GP, Local Authority, other connected Armed Forces Organisation, Regimental Association or other non-military support organisation. Once the form has been submitted it will be reviewed by our Trustees. We will email the agency with the progress of the request. The Agency must verify to say they have seen proof of service.

Each case will require a Head Office contact name and email address who will be copied into our decisions. (This must be different to the agency applicant)

Reason for the Phone request	When completing the reason please be as detailed as possible, i.e. Why the requestee cannot afford to buy a budget phone. The reasons leading to the hardship, the difference it will make and any current support packages in place.
Smart Phones can be provided, if the agency can contribute to them	We would be looking for at least 50% towards the cost of a smartphone on average around £35.00
